Corneal oedema after phacoemulsification surgery in diabetic and non-diabetic patients

Introduction: Patients undergoing cataract phacoemulsification surgery with comorbid diabetes mellitus are more susceptible to corneal oedema due to metabolic disorders that disrupt endothelial homeostasis. Corneal oedema is a common complication that occurs after phacoemulsification surgery. The presence of DM comorbidity is expected to prolong the recovery period for corneal oedema. Objective: To determine the relationship between DM and corneal oedema after phacoemulsification surgery.

Methods: A prospective cohort study with 180 samples was obtained using consecutive sampling techniques at RSI Sultan Agung Semarang and RSUD Dr. R. Soedjati Soemodiardjo Purwodadi from July 2023 to August 2023. The sample was divided into two research groups, the control group, and the DM group. Corneal oedema examination and sharp examination were carried out on the first day (D1) and the seventh day (D7) after phacoemulsification surgery. The relationship between variables is explained using a comparative test, the results are considered significant if p<0.05.

Results: The preoperative visual acuity in the non-DM group was significantly more severe than in the DM group (p=0.000). Post-operative visual acuity improved significantly in the non-DM group at post-operative D1 (p=0.02) and D7 (p=0.00) compared to the DM group. The degree of postoperative corneal oedema was significantly more severe in the DM group at post-operative D1 (p=0.004) and D7 (p=0.001) compared to the non-DM group.

Conclusion: Corneal oedema after phacoemulsification surgery accompanied by impaired visual improvement occurs more often in patients with DM risk factors compared to non-DM patients.
